If you see a walk area wrinkled, while the rest of the room is flat, what in your experience is the cause?

Cushion failure/lack of support from the cusion or improper stretch, water problem (cleaned improperly), or not properly power stretched.
However;;;; This is not a problem often seen.
More importantly:: WHat does one do to inspect this problem?

Cushion failure is so rare, I've only seen it once in 37 years. Of course there are a lot of cheater pads and some that are made really crappy, where one side of the roll is 1/16th of an inch at the edge and the other side of the role is 7/16ths of an inch.
Cleaning with water should not hurt the stretch,--unless done with hot water and the hot water screws-up the bond. It still shouldn't hurt the stretch, until the cleaner or another pulls the carpet back to dry it out.
Power stretching can cause just as many problems as it can cure. Why don't you as an inspector--- tell us how and why?
True cushion failure is a rarity.

Cushion failure includes loss of resilience.
Power stretching can cause problems by over stretching or under stretching excessively.
